Information on an independent Insurance broker and insurance brokerage
INFORMATION ON AN INDEPENDENT INSURANCE BROKER
Eurovalley s.r.o., IČO: 293 68 324, with its registered office at: Příkop 6, 602 00 Brno, is a registered independent Insurance broker at Czech National Bank in accordance with Act No. 170/2018 Coll., On the distribution of insurance and reinsurance. The broker is in accordance with Act no. No. 170/2018 Coll., on the distribution of insurance and reinsurance holder of a license for the activity of an independent Insurance broker 1,2 granted by the Czech National Bank, with its registered office at Na Příkopě 28, 115 03 Prague 1 ,
which supervises the activities of the Insurance Broker and its agents. The Broker and its agents are entered in a register kept by the Czech National Bank. The Customer can verify the registration of the Broker and his agents in the Czech National Bank register with the Czech National Bank at www.cnb.cz or by phone at +420 224 411 111.
The Broker will use the Czech language to communicate with the customer. In this language, the Customer will receive documents and other information from the Broker. The Customer may communicate with the Agent personally, by telephone in writing or by e-mail (email). The customer has the choice of whether the information provided by legal regulations by the Brokerr will be provided via paper records and reports or electronically via another durable medium. By signing this document, the Customer confirms that he has been offered both options to provide information and that he agrees to the provision of information by electronic means of communication (another durable medium) and that the chosen method is appropriate with regard to the insurance underwritten. Some information will be provided to the customer in accordance with the legal regulations through the Intermediary's website www.eurovalley.cz or through the website of the parent organization OK Group a.s. www.okgroup.cz, which the customer agrees to by signing this document. In this connection, the Customer declares that he / she has regular access to the Internet, uses this service regularly and has the opportunity to acquaint himself / herself with the information provided on the Broker's website and provided by electronic means of communication and that he / she has an established email address. If the customer does not have regular access to the Internet service, he / she is obliged to inform the Broker without delay. In the event that the customer requests the provision of information in paper form, he / she shall communicate his / her request to the Broker, who shall provide the information in this form to the customer.
The Broker issues rules for the identification and management of conflicts of interest (hereinafter referred to as the “Conflict of Interest Management Rules”) in accordance with the Act No. 170/2018 Coll., On the distribution of insurance and reinsurance. The objective of the Conflict of Interest Management Rules is effectively identifies and manages conflicts of interest in order to avoid damaging customers' interests. If the conflict of interests cannot be averted, the Broker shall always give priority to the interests of the Customer over the interests of the Customer or the interests of persons who are related to the Broker with the Property or otherwise. Incentives are a specific case of conflict of interest. In mediating insurance, the Broker or its Agents may not accept, offer or provide a monetary or non-monetary advantage that may lead to a breach of the obligation stipulated by the Act No. 170/2018 Coll., On the distribution of insurance and reinsurance or other legislation, including unusual remuneration for insurance mediation or any other unjustified financial, material or immaterial benefit. Neither the Broker nor his Agents have a direct or indirect share in the voting rights or capital of insurance companies which the customer can arrange insurance through the Broker. No insurance company which the customer may take out insurance wiht through the Broker or the person controlling the insurance company has a direct or indirect share in the voting rights or registered capital of the Broker or its Agents. More detailed information on conflicts of interest and incentives shall be provided by the Broker at the request of the Customer, which shall also be published on the Broker's website.
INFORMATION ON BROKING OF INSURANCE
The Broker mediates insurance for the customer as an insurance agent under a written contract with insurance companies, or as an insurance broker under a written contract with the customer, and may mediate insurance for insurance companies listed in the Czech National Bank register as cooperating insurance Broker and on the Broker website www. eurovalley.cz. The requirements, objectives and needs of the customer in relation to the insurance underwritten, the recommended insurance product, the reasons on which the Broker bases its recommendations so that the customer can decide whether or not to substantially change the insurance, and in the “Record of Customer Requirements, Needs, and Targets” document. The Broker and its Agents are remunerated in the form of commissions by the cooperating insurance companies in connection with the agreed insurance or change of insurance. Neither the Broker nor his Agents are remunerated directly by the customer. When mediating insurance, the Broker does not provide advice to the customer within the meaning of the provisions of Section 78 of the Code. When mediating insurance, the Broker shall provide recommendations pursuant to the provisions of Section 77 of the Code, except in the cases referred to in Article 77 (3) of the Code.
The Broker shall provide the Customer with regular reports on its services commensurate with the type of insurance products negotiated, including the costs associated with the provision of such services and transactions executed on behalf of the Customer. The Customer, the Insured or any other authorized person may file a complaint or complaint about the Broker’s conduct and its Agents in person, in writing or by email to the Brokers's contact details. These persons also have the possibility to contact the Supervisory Body of the Broker, which is the Czech National Bank - www.cnb.cz, or to file an action against the Broker at the local and local court, or to contact the extra-judicial body of dispute resolution Financial Arbiter - www.finarbitr.cz, for non-life insurance Czech Trade Inspection - www.coi.cz. The detailed procedure for submitting and settling a complaint or complaint addressed to the Broker is regulated by the Complaint Procedure Rules placed on the Broker's website.

2 As of 1 December 2018, the Company was the holder of an insurance broker registration pursuant to Section 8 of Act No. 38/2004 Coll. In accordance with the provisions of Section 120 (4) of the CNB's DPA, the Company shall register the Company after 1.2. 2019 to the CNB register as an independent intermediary with the same scope of activities.
3 As of 1 December 2018, the Company was the holder of an insurance agent registration pursuant to Section 7 of Act No. 38/2004 Coll. In accordance with the provisions of Section 120 (4) of the CNB's DPA, the Company shall register the Company after 1.2. 2019 to the CNB register as an independent intermediary with the same scope of activities.
